Housing information form for individuals- to be completed along with the household form HH001.
Instructions for use
Use one of these forms for each individual member of a household. It should be submitted to Ngati Rehia at the address on the form together with a single household form HH001 covering the whole household.
e.g. In a household with 5 members use 5 Ind001 forms and one 1 HH001 form.
